What is Patient Centred Care and why is it so hard to deliver? Why is it so difficult for patients to Connect with their clinicians? How do we empower and support patients to be actively engaged in their own healthcare journey?
Despite billions in investment and decades of software development the vision of Patient Centred, Connected Care remains mostly unrealised…until now. CRM software platforms that have transformed industry now have a growing role in the healthcare sector, rapidly filling capability gaps in traditional software systems.
Join our session to learn about our implementation of this new technology and how it might transform the future of health care delivery

Learning objectives:
  • Identify current challenges facing hospital systems providing high quality care with limited resources - costs, workforce shortages, COVID impacts, mental health burden, traditional approaches to system design/lack of innovation
  • Identify new approaches to using technology as a strategic tool to re-design healthcare provider models - low code-no code, drag-and-drop, platform vs off-the-shelf app, real-time AI/analytics, shift away from note-taking to true collaboration/coordination
  • Identify significant industry shifts in healthcare that are allowing us to change the way we deliver care - increasing consumer-centricity, community-based, decentralisation of care models

Biography

Alastair leads Heroiq: a Med Tech startup transforming digital patient engagement and remote patient monitoring, and an Emergency Physician at University Hospital Geelong. Passionate about lifelong education; he regularly provides expert insights to students, clinicians and industry in Digital Health and Critical Care. Alastair is an Affiliate Senior Lecturer at Deakin University, an Advanced Paediatric Life Support Instructor, and an Accredited Simulation Instructor with the NHET SIM Program. In the current climate emergency Alastair strongly advocates for environmental sustainability and rewilding: placing these values at the core of all his projects. He lives on the Surf Coast, Victoria with his young family, making the most of the beautiful landscape.

Senior Clinician Physiotherapist with Advanced Practice credentialing in patient care delivery and service improvement. Extensive senior experience providing research driven care across large tertiary hospitals, rural and regional health services. Leadership in clinical service implementation, extended scope training and University education programs.

Biography

Dr Jonathan Claydon is an Emergency Physician and Fellow of the Australasian College of Emergency Medicine. He graduated from Medicine & Surgery at Monash University (Clayton) with honours in 2008 where he was awarded the Bryan Hudson prize for top marks in Medicine. Jonathan commenced his training at The Alfred Hospital in Melbourne before moving to University Hospital Geelong to complete Emergency Medicine Training on the Surf Coast. Subsequently Dr Claydon worked at both Ballarat Base Hospital and University Hospital Geelong as a Staff Specialist Emergency Physician before consolidating his practice at UHG. Jonathan is also the Clinical Lead in Ultrasound in Geelong and helps drive an ever-increasing practice in Point-of-care Ultrasound and Regional Anaesthesia.
